When searching for a sperm bank, be sure to ask them about their quality commitment and how they will work with you, if that commitment is not met. As a sperm bank, Xytex Corporation prides itself on providing high-quality samples from healthy donors. Our quality commitment is 15 million motile sperm per sample (0.5mL), and post-thaw motility must be at least 30 percent—the highest quality commitment in the industry. It is a rare occurrence that Xytex sperm bank receives complaints about sample quality, but if you feel you’ve received a sample that is substandard, you should take the following steps.
Within 30 days of the thaw date, please contact us and we will provide your physician with a lab report form. This lab report asks for the donor number, lot number, patient name (if the patient was invoiced for the shipment), thaw date, and the sample evaluation prior to any further processing. The sample evaluation should include the count, motility and kinetics. A Credit Memo will be written and the lab report will be reviewed by our clinical operations director. A phone call to the physician may be necessary to obtain further information. If the sample is found to be below our quality commitment, Xytex will provide you with a replacement sample.
We try to complete the Credit Memo process as quickly as possible. To ensure a timely response to your request, be sure to provide Xytex sperm bank with the lab report as soon as possible. Remember, a report of a poor-quality sample must be sent to Xytex sperm bank within 30 days of the thaw date in order to be considered for replacement. Also note that any further processing of the samples by your doctor voids the Xytex quality commitment.
