When the exciting time comes to place your order, you can do so by calling 800-277-3210 or e-mailing xytex@xytex.com. We cannot ship any order until we have a signed Supply Agreement on file for your doctor (downloadable at http://www.xytex.com/sperm-donor-bank-physician/index.cfm#supply). If you wish for samples to be sent directly to your home rather than your physician’s office, you must have your doctor fill out a Home Shipment Authorization form (also downloadable from the website).
So why is a doctor’s authorization required? Here’s the explanation:
It is required by law in the state of Georgia, where Xytex processes semen, and also by New York State, which has regulatory authority over Xytex. A tissue bank in Georgia is regulated as a clinical diagnostic laboratory that is allowed to provide services only to qualified professionals. To comply, Xytex actually confirms credentials with the professional jurisdictional licensing authority, and then reconfirms the gamete order with the patient’s professional. This law is consistent with medical ethics; since the patient is seeking professional involvement in achieving pregnancy and the clinical process is subject to some risk, the patient deserves professional counseling. Technical nuances of clinical insemination and cryogenics are beyond common knowledge of most patients.
